Blue Card

Express Lab is pleased to announce the Blue Card program to facilitate patients. Cardholders can avail discounts at Express Lab and other participating healthcare establishments. In addition, cardholders can earn points that may be redeemed at Express Lab at a later time.

How to get your Blue Card?

At the time of the Blue Card request, please provide the following information to the Express Lab representative in order to process your Blue Card:

• Name
• Contact no
• Date of birth
• Postal address
• Email address

Facilities on Blue Card:

• 20% off on Lab test

Validity of Blue Card:

The Blue card will be valid for a period of 2 years from the date of issuance. You may renew it at any time against a payment of Rs. 200. In case of loss, please call us to get a replacement Blue Card.

Types of Blue Cards:

• Family Blue Card - Rs. 700/-
  (9+1 maximum of 10 members including the Blue card holder)
• Conversion of a single card into a family for Rs. 500/-

Terms and Conditions:

• Blue Card is non-transferable. Only Blue Card members listed on the card account may use the Blue Card.
• Blue Card benefits at healthcare providers other than Express Lab are at the sole discretion of that provider.
• Express Lab shall bear no liability if a third-party healthcare provider opts out of honoring Blue Card.
• If you lose your Blue Card, please notify us immediately so that a replacement card may be issued. Replacement card fees will apply.
• Express Lab reserves the right to alter/discontinue the Blue Card program or individual Blue Cards at any time at its sole discretion.
• Special sales and promotions may not be eligible for discounts. Only one offer can be availed of at a time.
• Express Lab may decide not to offer/discontinue discounts on certain tests/procedures.
